Insurance Information
We participate with multiple insurance carriers and multiple different policies. If you have a question about your insurance please call our office at 586-751-2520 option 2 to speak to someone in our billing department.

We do not participate with any Medicaid programs.
Underage Patients
Any patient under the age of 18 years old is considered a minor. Minors must be accompanied by a parent or legal guardian for his/her initial appointment.

Return appointments do not require a parent or legal guardian to be present unless a treatment or procedure that requires legal informed consent is being performed.
​
If a treatment or procedure is found to be necessary and a parent or legal guardian is not present the treatment or procedure may be delayed until legal informed consent may be obtained.
Patient Portal
We utilize a secure website to provide our patients with 24-7 access to the electronic medical record and personal healthcare information.

In order to access your patient portal you must provide us with a valid e-mail address that we will utilize to activate your portal. After your portal has been activated you will receive an e-mail with log in instructions. After you have logged in for the first time and established your user name and password you will be able to return to the portal and log in whenever necessary.
